What to review
Review the complete approach, ladder or stair, landings, gates, openings, transitions, and exit. Keep access clear of materials, hoses, cords, debris, doors, vehicles, and operations. Use access components intended for the exact scaffold and current instructions.
Review the exact scaffold, visible condition, complete configuration, current instructions, task, environment, access, surrounding operations, and changes since the last authorized review. Use the site-specific process
Record the scaffold identity, findings, restrictions, changes, corrective action, and responsible role using the employer or facility procedure. Stop and escalate when a component, condition, configuration, instruction, or authorization is uncertain.

Connect inspection, education, and equipment decisions
Inspection, training, tagging, manufacturer information, site controls, and equipment selection serve different purposes. A completed checklist or tag communicates information; it does not make the underlying condition acceptable.

Begin the scaffold access requirements review with evidence rather than assumptions. Record the exact scaffold family, model and component markings; retain the current manufacturer instructions; identify the applicable sections of the current OSHA standard; and document the work activity, environment, dimensions, loading inputs, access route, surrounding operations, and responsible personnel.
